Our services are provided by licensed healthcare professionals under applicable Texas and federal law.

• Be at least 18 years of age, or have a parent or legal guardian consent on your behalf
• Reside in or near the service area (Houston, TX metropolitan area) for in-person or mobile services
• Provide accurate and complete registration and health information
• Agree to receive care in accordance with our clinical protocols and provider recommendations

You agree to use our website only for lawful purposes and in a manner that does not infringe the rights of others. You agree not to:

• Use the website in any way that violates applicable local, state, national, or international law

5. Appointment Scheduling and Cancellations

Appointment requests submitted through our website or patient portal are subject to provider availability and confirmation. An appointment is not confirmed until you receive confirmation from our office by phone or email.

Cancellations and No-Shows

We ask that you provide at least 24 hours notice if you need to cancel or reschedule your appointment. Repeated no-shows or late cancellations without notice may result in a cancellation fee or discharge from the practice.

Mobile Home Visit Policy

Home visit appointments require a confirmed address and a responsible adult present during the visit. CDIVINE reserves the right to decline or terminate a home visit if the environment presents a safety risk to our providers or staff.

Our telehealth services are available to patients located in Texas at the time of their virtual appointment. By participating in a telehealth visit, you acknowledge that:

• Telehealth has limitations and may not be appropriate for all conditions
• Your provider may determine that an in-person visit or referral is necessary
• You are responsible for ensuring a private, secure connection during your visit
• Technical difficulties may occasionally disrupt or delay your appointment

7. Payment and Insurance

CDIVINE Medical House Services accepts most major insurance plans, Medicare, and Medicaid. It is your responsibility to verify your coverage and benefits before receiving services. You are responsible for all applicable co-pays, deductibles, and any charges for non-covered services.

Self-pay patients must make payment arrangements prior to or at the time of service. We offer flexible payment options — please contact our billing department for details.

8. Patient Portal

Access to our patient portal is a privilege. You are responsible for maintaining the confidentiality of your login credentials and for all activities that occur under your account. You agree to notify us immediately of any unauthorized use of your account. Do not share your login credentials with others.
